Each person sees only their own record.When a memo names two people, each moment calls the other person “a housemate”.
Not every hard moment is an incident.An argument over the TV remote is marked follow up, not incident, because no harm was described. Over-reporting buries the incidents that matter. A team leader always makes the call.
Gaps are shown, not filled.“No moments recorded between 10:41 and 19:20” is useful to know. The progress note lists what wasn't recorded today instead of guessing.
